Case 4

This patient is concerned by dark marks present in the axillae and groins.

Erythrasma

Infection with a gram-positive organism, Corynebacterium minutissimum.

The diagnosis is confirmed by noting coral-red fluorescence on Wood's light (long wave UVA) examination. Microscopy and culture will identify the bacteria responsible, if the laboratory is informed of the clinical suspicion.

Erythrasma responds to topical imidazole or fucidic acid creams. Oral erythromycin is also effective.
